referral form below is a request for a referral!
If you believe you need a referral, before filling out the form, you should first go to the Referrals page and read the online material. It may be that upon review, your doctor will call and request
that you be seen in the office, or suggest another plan of approach. This is particularly useful when you are seeing a referral already and you need more visits or procedures. Expect 72-hour turnaround by the
time it is reviewed by the referral staff, your doctor, the insurance company permission is granted, and the referral is either faxed back to you or the doctor. If you need more than one referral...
